Left Thalamic Bleed
Here's a nasty bleeder from a 62 year old with no past medical history of hypertension but a typical location for a hypertensive bleed. She had some embolic looking changes in her toe that were worrisome for possible cardioembolic phenomenon but I would think that thalamus is an unlikely locale for emboli which usually find their way to the gray-white junction. Unfortunately patient was intubated, extubated and then re-intubated so no opportunity for good exam findings. Perhaps we'll take this opportunity to review the anatomy of the thalamus at some point.
